VOLUNTEER SERVICE AWARD 2009

 

 

APPLICATION DEADLINE: APRIL 8, 2009

 

The Optimist Club of Deerfield offers several scholarship awards to students selected for its “Volunteer Service Award.”  This program is designed to recognize volunteer service activities and is not based on grade attainment or financial need.  Awards may range up to $5,000 to a single recipient.

 

The following requirements are effective for this award year.

 

1.      A qualified student must be living in an area where he/she would be eligible to attend Deerfield High School and participates in volunteer service to the community.

 

2.      The applicant must currently be a senior in an accredited high school program, or an approved home school program.

 

3.      “Service to the Community” is defined as a voluntary, uncompensated effort benefiting a non-profit organization or individuals.

 

4.      The applicant is required to submit a narrative describing his/her activities, experiences and the impact of their volunteer service. Additional information may be found on the application.

 

5.      Applicants must submit a letter of recommendation from the organization that supports their volunteer service.  The letter(s) of recommendation must be in original sealed envelope(s) with the adult sponsor’s signature across the seal.  They may be sent directly to the Optimist Club or they may be included in your application package.  Please note that if sent directly, the letters must be postmarked no later than April 8th, 2009. 

 

6.      The recipient of an award must show evidence of acceptance for post secondary education such as a junior college, traditional college/university, trade or vocational education or training.

 

7.      The Scholarship Committee of The Optimist Club of Deerfield is the only judge of these awards and all decisions are final.

 

8.      Transcripts and financial information are not required for this application.

 

9.      The application form, letters of recommendation and narrative(s) must be postmarked no later than April 8, 2009 and sent to:
